A groundbreaking study has brought new hope to women and their twins affected by a rare and life-threatening condition known as twin-twin transfusion syndrome (TTTS). This condition, affecting around 300 to 400 pregnancies annually in the UK, occurs when identical twins share a placenta and one twin receives too much blood while the other receives too little, potentially leading to organ failure and even death. The study, led by Professor Christoph Lees and published in the American Journal of Obstetrics and Gynaecology, introduces a non-invasive procedure using high-intensity focused ultrasound (HIFU) to treat TTTS, marking a significant advancement in maternal and fetal healthcare.

The research, funded by the Medical Research Council (MRC) and involving UK and European patients, is a world-first for this technique in TTTS. The procedure involves using a Doppler ultrasound to identify the tiny blood vessels in the placenta that cause TTTS, and then applying a beam of high-energy sound waves to precisely heat and block blood flow through these vessels. This non-invasive approach successfully blocked blood flow in 90% of the treated placental blood vessels, with no adverse side effects reported.

Current treatments, such as inserting a fetoscope and using a laser to block blood flow, carry risks of miscarriage and require a small incision in the mother's stomach. Brioney Garrett, who participated in the trial, shared her story, highlighting the limited options and the relief of finding a safe and effective treatment.

While the study did not prove clinical efficacy or survival rates, it demonstrated the potential of HIFU as a viable and safe treatment option. The team now plans to conduct a larger clinical trial with 50 to 100 women to further evaluate the procedure's effectiveness and safety.
